Anticoagulant Therapy Packs

Should a patient’s first oral anticoagulant therapy appointment be dominated by filling forms, cards and record books? Should consultant time be wasted on paperwork? Should patients be asked to complete their own records due to time constraints? We believe there is a better way, one which removes the administrative burden, reduces transcription errors and allows time to be refocused back to the patient.

We have developed an alternative solution to the traditional ‘Yellow book’ system which removes the need to handwrite or label patient information and record books. It works with existing software solutions and laser printers and needs only minimal investment in a pressure seal machine to seal ongoing patient dosage letters.

The Prolog Print Media oral anticoagulant therapy pack and treatment record solution consists of four parts:

• Part one – Yellow folder
  A folder designed to carry the three pieces of information most vital to patients undergoing oral anticoagulant therapy: their contact information, advice booklet and treatment record.
• Part two – Form A1
  An A4 printable patient details form with printed pop-out patient alert card.
• Part three – Important information advice booklet
  A pre-printed 12 page booklet to communicate to patients key advice about oral anticoagulant therapy.
• Part four – Treatment record pouch
  A pouch for patients to store their latest treatment results and dosage information:  presented as a pressure seal printed form.

All four pieces of the pack are available in minimum quantities of 500 and priced at only a few pounds per patient we believe it will be money well spent.
